About

Who We Are

Grief

Our goal is to help people live their final months as fully and peacefully as possible. High quality healthcare is important at every stage of life and we consider it a privilege to walk alongside those who are facing terminal illness, coping with grief, or seeking to learn more about these profound experiences.

Northland Hospice & Palliative Care, a community resource since 1983, provides hospice care for people of any age with any terminal diagnosis. Additionally, our palliative care service specializes in relief of pain, symptoms, and stress at any point in an illness.

We Provide:

  • A team approach. Our medical director, nurses, aides, social worker, chaplain, and volunteers work together to meet the needs of each patient and family. We work in conjunction with your doctor and others providing care.
  • An affordable option. Hospice costs are covered under Medicare, Medicaid, most private insurance plans, and managed care organizations. Sliding-scale fees are also available. No one is ever turned away for inability to pay.
  • Care in the setting that is best for you. This may be a private home, hospital, or nursing home. We also have a 10 bed residence in Flagstaff.
  • Support for those who are grieving. Our staff and volunteers work with family members of all ages after the death of a loved one. We provide counseling services, educational programs, and support groups to anyone in the community.

Our Mission:

Northland Hospice and Palliative Care is a non-profit community based organization committed to excellence in end-of-life and comfort care.

These missions are accomplished through our commitment to:

  • Provide care in the patient’s home environment in accordance with the NORTHLAND HOSPICE & PALLIATIVE CARE Statement of Patient and Family Rights.
  • Assist the patient and family with mental and spiritual preparation for death that is meaningful to them.
  • Respond to the needs of the patient and family through interdisciplinary teams that direct care and promote comfort and a supportive environment.
  • Assure a highly qualified and specially trained team of hospice professionals and volunteers who work together to provide compassionate care to meet the physiological, psychosocial, spiritual and economic needs of hospice patients and families.
  • Provide an administrative framework responsible for implementation of organizational policies in accordance with external regulations.
  • Maintain a documentation system which assures the quality of the program and it’s services.
  • Create and foster community awareness and participation in support of hospice philosophy, programs, services and resources through a sustained public information and education program.
  • Maintain fiscal responsibility through generating resources that support the organization’s mission.

Our Vision:

Northland Hospice and Palliative Care, a non profit organization, is the first choice of professionals and families to provide the highest quality hospice and palliative care services in Northern Arizona, serving those in need of services regardless of ability to pay.

Northland Hospice and Palliative Care is recognized for it’s premier services, outstanding leadership, and integrity, and is supported by the community.
