Registered Nurse (Job Posting 1 of 2)
NATURE AND SCOPE OF POSITION: Serves as member of an interdisciplinary team and provides skilled nursing care to the terminally ill and their families in a home setting through systematic assessment of health needs in accordance with the stated Mission of Northland Hospice and Palliative Care and policies and procedures.
ESSENTIAL DUTIES
- Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
- Makes patient assessments, teaches, supervises, and counsels the patient, family members and staff regarding the care and other related standards
- Assesses patients and identifies needs that encompass the total well-being of the patients
- Instructs the patient/family about hospice services and the individualized plan of care
- Develops an effective plan of treatment in collaboration with the referring, attending physician, medical director and multidisciplinary team.
- Uses nursing process in providing exceptional patient care
- Documents all pertinent information appropriately
- Observes, records and reports to the physician any reaction to treatment or changes in the client’s condition
- Demonstrates courteous, cooperative, respectful attitude with patients, families and co-workers
- Maintains compliance with HIPAA requirements as per organization policies
- Serves as a Northland Hospice and Palliative Care nursing representative for outside organizations and institutions
- Serves as Liaison between patient, family, primary physician and entire hospice team regarding the therapeutic plan as prescribed by the physician
- Participates in Continuous Quality Improvement activities
- Assists in the education of medical students/residents, nurses, nursing assistants, volunteers and others as identified
- Provides consultation and assistance, as needed, in the coordination of final 24 hour care
- Assumes compliance with State and Medicare regulations

EDUCATION AND/OR EXPERIENCE
- Graduate of an accredited School of Nursing for Registered Nurses
- Minimum of one year previous RN experience
- Previous Hospice experience preferred
- Graduate of Northland Hospice and Palliative Care Volunteer Training Program

CERTIFICATES/LICENSES/REGISTRATIONS
- Reliable transportation, current driver’s license, and current auto insurance
- Possess a current and valid Fingerprint Clearance Card
- Current and valid American Heart Association CPR Certification preferred or other in person certification course
- Possess a current license as a Registered Nurse in the state of Arizona
Experienced Caregiver (Job Posting 2 of 2):
- Must have CPR, 1st aid, Finger Print Clearance card; PT/PRN with the potential to go FT Weekends/Nights
