Policy & Procedure on Patient Privacy Rights

POLICY

Northland Hospice & Palliative Care implements policies and procedures to accommodate patient privacy rights as required by and specified in the Privacy Rule of the Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996. 

PROCEDURE 

  • Patients cared for by Northland Hospice have the following rights with respect to the privacy of their health information:

  • To receive a paper copy of the hospice’s Notice of Privacy Practices

  • To lodge complaints about the hospice’s privacy practices

  • To request restrictions on the uses and disclosures of health information

  • To request to receive confidential communication

  • To access their protected health information for inspection and/or copying

  • To amend their health care information

  • To request an accounting of disclosures of health information

  • The privacy policies of Northland Hospice detail the requirements for each of these rights and provide procedures for implementation
     

  • Staff at Northland Hospice are provided with annual training regarding patient rights with respect to their health information
     

  • Information related to patient privacy rights is included in the orientation program for new staff.
     

  • The hospice’s Patient Rights and Responsibilities Statement includes patient privacy rights.
